Respirator Medical Questionnaires
To ensure that wearing respiratory protection doesn’t cause undue health concerns, OSHA requires you to “provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace.”
After the questionnaire has been reviewed by the Physician we will provide you with a written document summarizing the physician’s opinion.
Option 1) Online
We have a unique web-based (mobile friendly) Respirator Medical Evaluation portal. It is a fast and convenient way to conduct your employee’s respirator medical questionnaires.
Option 2) Onsite
We can provide paper based respirator medical questionnaires to your employees so they can complete them at the jobsite.

How long does it take to complete the questions? On average, it will take approximately 15-20 minutes to complete the questionnaire.

How long does it take to get the results?
Clearance results will be available within 2-4 business days. Our online and paper based questionnaires are reviewed by an independent Occupational Physician (MD) not a computer algorithm.

What if I am not approved?
Most people are approved however there are certain medical conditions that may require further in person evaluation including pulmonary function testing, spirometry, etc…. If your medical clearance is not approved you will have to find a suitable clinic to conduct the in person assessment. Safety Links does not provide this service.

What documentation do I receive after the Doctor reviews the questionnaire?
After the questionnaire has been reviewed by the Physician we will provide you with a written document summarizing the physician’s opinion. You can print or forward the email to your Safety or HR department.

What is the cost to do the Medical Assessment?
We charge $57 to do the assessment however for larger groups the price can be negotiated.

Why is Safety Links slightly more expensive than other online respirator approval companies?
All of our questionnaires are reviewed by an independent Occupational Physician (MD) not a computer algorithm. The other 2 companies that do this service provide instant results. Although this sounds great this means that a Doctor has not reviewed your questionnaire. Without having the direct involvement of the Doctor the other providers are simply not compliant.
